Efficacy of low frequency repetitive transcranial magnetic stimulation as an adjunctive treatment to zolpidem in patients of primary insomnia: a randomized controlled single blinded study

Abstract
ObjectiveTo explore the efficacy and safety of low frequency repetitive transcranial magnetic stimulation (rTMS) as an adjunctive treatment to zolpidem in patients of primary insomnia, compared with zolpidem monotherapy.
MethodsSixty patients aged between 18 and 55 years, who met the DSM-Ⅳ criteria for primary insomnia, were randomized to either active rTMS (research group, 1 Hz and 90% motor threshold stimulation at right dorsolateral prefrontal lobe) or sham treatment (control group) for 4 weeks. Patients were also assigned to receive zolpidem treatment (5-10 mg/d) for both groups. Polysomnogram (PSG) and the Pittsburgh Sleep Quality Index (PSQI) were measured at baseline and the end of study.
ResultsAfter treatment, the sleep onset latency, wakeup after sleep onset, and total sleep time, measured by PSG in the research group ((28.60±4.60), (80.65±28.13) , (389.75±33.71) min) were significantly improved than the control group ((34.90±3.81), (120.39±33.48), (336.61±34.26) min; t=-2.334, -2.412, 2.383, all P<0.05) . PSQI total score was significantly lower in the research group (t=-2.212, P<0.05). In addition, the sleep efficacy, Ⅲ sleep and rapid eye movement in the research group were also significantly improved than the control group (t=2.132, 2.238, 2.368, all P< 0.05).
ConclusionThe efficacy of low frequency rTMS stimulation combined with zolpidem is superior to zolpidem monotherapy in patients of primary insomnia.
