Feasibility of radical operation in patients with stage Ⅲ pancreatic carcinoma

Abstract
ObjectiveTo study the role of radical operation in patients with stage Ⅲ pancreatic carcinoma and to improve the radical resection rate of pancreatic carcinoma.
MethodsWe retrospectively analyzed the clinical data of 44 patients with stage Ⅲ pancreatic carcinoma who underwent radical operation at the PLA General Hospital between January 2009 and December 2013. The general data, postoperative complications and follow-up information between the radical group and a control group were statistical analysed. We selected 55 stage Ⅲ pancreatic carcinoma patients who did not receive any surgical treatment as the control group during the same period. The survival rates of the two groups were compared.
ResultsThere was no death in the peri-operation period. The most common postoperative complication of the radical group was pancreatic fistula and the rate was 22.7% (10/44). The incidences of post-operation complication and pancreatic fistula in the radical group were significantly higher than the control group (P<0.05). Radical operation did not increase the incidence of Clavien Ⅲ stage complications. The follow-up end point was in September 2014, the follow-up was 9 to 69 months. The total follow-up rate was 90.9% (90/99). The survival rates for patients in the radical group at half year, one year, two years and three years were 65.0%, 41.9%, 19.4% and 14.6%, respectively. The mean±SD survival was (16.0±1.2) months. The survival rates for patients in the control group at half year, one year, two years and three years were 58.0%, 14.6%, 8.3% and 0%, respectively. The survival (mean±SD) was (9.3±0.8) months. The Log-Rank test showed that the survival rate and the mean survival time in the radical group were significantly higher than the control group (P<0.05).
ConclusionsThe radical operation is still an effective treatment for patients with stage Ⅲ pancreatic carcinoma. The operation effectively prolonged the survival time of patients and did not increase the incidence of Clavien Ⅲ stage complications.
