Predictive value of D-dimer quantitative determination on postpartum hemorrhage with hypertensive disorder in pregnancy

Abstract
ObjectiveTo investigate the predictive value of D-dimer quantitative determination on postpartum hemorrhage with hypertensive disorders in pregnancy patients.
MethodsOne hundred and fifty cases with hypertensive disorders in pregnancy admitted to Chaozhou Hospital from January 2013 to December 2013 were retrospectively analyzed. D-dimer data of all the 150 patients were collected at the 24 hours before delivery. The patients were divided into postpartum hemorrhage group and control group. The correlation between the D-dimer measurement and a loss of blood in the postpartum period was analyzed by one-way ANOVA, and the accuracy of D-dimer predictive variables of postpartum hemorrhage was analyzed by ROC curves.
ResultsTwenty-eight cases had postpartum hemorrhage, and the incidence rate was 18.67% (28/150). The average value of D-dimer was (9.56±11.97)mg/L in the postpartum hemorrhage group, while the average value of D-dimer was (1.96±1.06) mg/L in the control group, with a significant difference (F=48.61, P<0.01). The area under the ROC curve was 0.848 (95% CI: 0.766-0.930), standard error was 0.042. The optimal threshold was 2.26 mg/L, whose specificity, sensitivity and Youden index were 66.4%, 92.9% and 0.593, respectively.
ConclusionQuantitative determination of D-dimer has a significant role in predicting postpartum hemorrhage on hypertensive disorders in pregnancy patients, the optimal threshold is 2.26 mg/L.
