The risk factors of intravesical recurrence after upper urinary tract urothelial carcinoma and the significance of preventive intravesical chemotherapy

Abstract
ObjectiveTo investigate the risk factors of intravesical recurrence after upper urinary tract urothelial carcinoma and the significance of preventive intravesical chemotherapy.
MethodsThe clinical data of 53 patients with upper urinary tract urothelial carcinoma in our hospital from May 2011 to November 2016 were analyzed retrospectively, according to recurrence, patients were divided into recurrent group and non-recurrent group, the pathological data of two groups were compared, and two groups of patients were followed up, the risk factors for recurrence of bladder cancer were analyzed, and the average survival time of two groups was compared.
ResultsPatients with bladder perfusion for recurrence group was significantly lower than that of non-recurrence group, there was statistically significant difference(P<0.05), perfusion patients with an average survival time was slightly higher than the non-reperfusion patients, but the difference was not statistically significant(P>0.05). Open surgery was adopted in 7 of the 53 cases, and laparoscopic surgery was adopted in 46 cases, there was statistically significant in age, tumor location, number of lesions, surgical method, tumor grade, prophylactic instillation of bladder difference between the recurrent group and non-recurrence group(P<0.05). Logistic regression analysis showed that age, surgical method, tumor grade, prevention of bladder perfusion and postoperative recurrence of bladder cancer, there were significant correlation(P<0.05), and age, surgical methods and tumor levels were risk factors for postoperative recurrence, preventive bladder perfusion was a protective factor for the prevention of postoperative recurrence of bladder cancer.
ConclusionsThe age, surgical method and tumor level of the patients with upper urinary tract urothelial carcinoma can be used as a risk factor to determine the recurrence of bladder cancer after ureterectomy, prophylactic intravesical instillation can effectively reduce the recurrence of bladder cancer after upper urinary tract urothelial carcinoma.
