Meta-analysis of genetic therapy for peripheral arterial diseases

Abstract
BackgroundTo evaluate the safety and efficacy of genetic therapy for intractable peripheral arterial disease (PAD).
MethodsRandomized controlled trials (RCTs) refer to genetic therapy for PAD before October 2019 were retrieved from four database including PubMed, Embase, ISI Web of Science and Cochrane Library. According to the eligibility criteria, the retrieval literature was screened, the quality evaluation and data extraction were conducted. RevMan 5.3 software was used for meta analysis.
ResultsA total of 19 articles involving 2 104 pattients were included. Meta-analysis showed that the effects for genetic therapy on major limb amputations (RR=0.93, 95%CI: 0.74~1.17, P=0.56; 13 studies, n=1 371) and death (RR=0.93, 95%CI: 0.70~1.24, P=0.64; 17 studies, n=2 051) at up to two years were uncertain. However, genetic therapy might decrease the risk of any amputation by 37% (RR=0.67, 95%CI: 0.47~0.94, P=0.02; 8 studies, n=489), improve ulcer improvement by 148% (RR=2.48, 95%CI: 1.40~4.41, P=0.002; 5 studies, n=199), and improve rest pain by 94% (RR=1.94, 95%CI: 1.11~3.37, P=0.02; 6 studies, n=213).
ConclusionFor intractable PAD, genetic therapy may decrease the rate of any limb amputations, and improve ulcer complete healing.
