Hemodialysis service in community health service centers supported by medical partnership

Abstract
Sunhe Community Health Service Center is the member of medical partnership of China-Japan Friendship Hospital. According to the real needs of local patients, the center started hemodialysis service on January 2017 with the help of superior partnership hospital. The responsible doctor from the China-Japan Friendship Hospital provided assistance in staff training, facility construction, technical guidance and clinical supervision; and the superior hospital also offered a green channel for patients` referral from the community center. Since starting hemodialysis service with this model, 73 local patients received total 16 768 hemodialyses up to January 2020. During this period 82 patient/times were transferred to superior hospital due to vascular access dysfunction, secondary hyperparathyroidism and other conditions, and referred back after appropriate treatment. This cooperative model makes full use of medical resources and expands the service scope of community health service centers; provides easy access for local chronic renal failure patients, and also improves the clinical capacity of primary care medical staff with sense of professional honor.
