Value of transcranial Doppler ultrasonography substantia nigra echo characteristics in differential diagnosis of different types of Parkinson’s disease

Abstract
ObjectiveTo analyze the value of transcranial Doppler ultrasonography substantia nigra echo characteristics for differential diagnosis of different types of Parkinson’s disease.
MethodsSixty patients with Parkinson’s disease who were treated in Zhengzhou Yihe Hospital from March 2018 to April 2020 were selected as the observation group; and 58 patients with primary tremor who received treatment during the same period were selected as the control group. All patients underwent transcranial Doppler ultrasonography, and the total area of bilateral midbrain, total hyperechoic area of bilateral substantia nigra (SN), and total hyperechoic area of substantia nigra/total midbrain area (S/M) between the two groups were recorded and compared; according to the different types of Parkinson’s patients, they were divided into tremor type, tonic type and mixed type. The SN high echo area and S/M of each group, drew receiver operating characteristic (ROC) curve, and the diagnostic value of echo characteristic of SN and S/M for different types of Parkinson’s disease were tested.
ResultsThe total area of bilateral midbrain was compared between the observation group and control group, there was no significant difference (P>0.05); the SN total hyperechoic area and S/M of the observation group were greater than those of the control group (P<0.05); among the 60 patients with Parkinson’s disease, there were 25 cases of tremor type, 23 cases of tonic type, and 12 cases of mixed type; the mixed group had the highest SN echo area and S/M, followed by the tonic type and tremor type (P<0.05); drew ROC curve, SN high echo area, S/M area under the curve (AUC) for diagnosis of different types of Parkinson’s disease were all>0.80, the predictive value was ideal; the tremor type and tonic type SN high echo intensity classification were concentrated in grades Ⅰ-Ⅲ, while the mixed type was mainly concentrated in grades Ⅳ-Ⅴ; in addition, mixed-type Parkinson’s disease had the highest SN high echo positive rate, which was higher than that of tremor type and tonic type (P<0.05), and there was no significant difference between tremor type and tonic type (P>0.05).
ConclusionsThe substantia nigra echo characteristics of different types of Parkinson’s disease are different, clinically, different subtypes of Parkinson’s disease can be identified by observing the characteristics of transcranial Doppler ultrasonography substantia nigra echoes, in order to improve the accuracy of differential diagnosis of different types of Parkinson’s disease.
