Abstract
With the development of social economy, under the background of population aging and migration of chronic diseases, chronic diseases, with their characteristics of high morbidity, high disability rate, long course of disease and high mortality, have a serious impact on human physical and mental health as well as social and economic development.Due to the contradiction between people's growing health needs and the unequal distribution of medical resources, the inadequacy of chronic disease management is becoming increasingly prominent. With the continuous development of the Internet of Things technology, it directly penetrates into the medical field and changes the prevention and control of chronic diseases.Based on the introduction of the national hierarchical diagnosis and treatment system, the establishment of scientific and effective chronic disease prevention and control strategies by using the Internet of Things technology not only promotes the reasonable allocation of medical resources, but also provides continuous, high-quality and whole-course chronic disease management services for patients with chronic diseases. This paper focused on the construction of a new community health and behavior service management strategy for chronic patients based on the modern Internet of Things and information technology environment, and discussed its research content and construction ideas, aiming at providing references for the social health management of chronic diseases.
