Risk factors of hospital acquired carbapenem-resistant Klebsiella pneumoniae pulmonary infection in intensive care unit

Abstract
ObjectiveTo understand the risk factors of hospital acquired carbapenem-resistant Klebsiella pneumoniae (CR-KP) pulmonary infection among patients in intensive care unit (ICU).
MethodsThe clinical data of patients with hospital acquired Klebsiella pneumoniae pulmonary infection in ICU of Jiaxing First Hospital from 2016 to 2018 were retrospectively analyzed. According to the resistance of Klebsiella pneumoniae to carbapenems, the patients were divided into CR-KP infection group (52 cases) and carbapenem-sensitive Klebsiella pneumoniae(CS-KP) infection group (104 cases). Risk factors associated with CR-KP infection were analyzed by multivariate unconditional Logistic regression analysis.
ResultsMultiple Logistic regression analysis showed that the length of ICU stay before infection ≥7 d (OR=3.68, 95%CI: 1.69-5.12), diabetes (OR=2.98, 95%CI: 1.79-6.78), acute physiology and chronic health evaluation(APACHEⅡ) score > 18 ( OR=3.51, 95%CI: 2.71-6.57), application of carbapenems(OR=2.69, 95%CI: 2.08-8.98), mechanical ventilation(OR=2.34, 95%CI: 1.18-3.07) and indwelling catheter (OR=2.32, 95%CI: 1.73-3.60) were risk factors for hospital acquired CR-KP pulmonary infection.
ConclusionsIn order to reduce the incidence of hospital-acquired CR-KP pulmonary infection, it is important to properly control patients’ blood glucose, reduce the time of ICU stay, rationally use carbapenems, as well as reduce mechanical ventilation and urinary intubation according to the condition of patients.
