Analysis of influencing factors of postoperative recurrence of perianal abscess treated by radial multi-incision floating line drainage

Abstract
ObjectiveTo investigate the related influencing factors of postoperative recurrence of perianal abscess treated by radial multi-incision floating line drainage.
MethodsThe clinical data of 152 patients with perianal abscess aged 18-65 treated in our hospital from January 2018 to January 2020 were retrospectively analyzed, including 99 males and 53 females. All the patients underwent radial multi-incision floating line drainage. The recurrence of perianal abscess was followed up after operation. The general clinical data before operation, the use of postoperative antibiotics, and the influence on postoperative recurrence were statistically analyzed.
ResultsThere were 48 cases of postoperative recurrence of perianal abscess, with the recurrence rate of 31.58%. There were no statistically significant differences in age, body mass index (BMI), drinking history, abscess location, and abscess type between the recurrence group and the non-recurrence group (all P>0.05). The rates of male, smoking history, deep abscess, perianal abscess history, and diabetes mellitus in the recurrence group were 77.08%(37/48), 70.83%(34/48), 27.08%(13/48),64.58%(31/48), and 62.50%(30/48), which were significantly higher than those in the non-recurrence group [59.62%(62/104),48.08%(50/104),6.73%(7/104),37.50%(39/104), and 38.46%(40/104)], the rate of using antibiotics was lower than that in the non-recurrence group [33.33%(16/48) vs.57.69%(60/104)] (allP<0.05). Logistic regression analysis showed that smoking history, deep abscess, perianal abscess history, and diabetes mellitus were risk factors for postoperative recurrence of perianal abscess, and the use of antibiotics was a protective factor for postoperative recurrence of perianal abscess (allP<0.05).
ConclusionsPatients with perianal abscess history, smoking history, deep abscess, and diabetes mellitus will increase the risk of postoperative recurrence of perianal abscess. The use of antibiotics can reduce the risk of postoperative recurrence. In the clinical treatment of perianal abscess, we should choose targeted therapeutic regimen according to the patients' past medical history to reduce the risk of postoperative recurrence of perianal abscess.
