摘要

目的探讨血液透析过程中跨膜压高报警的原因及相应处理方法。

方法对2016年6月468例次血液透析患者进行观察,在透析过程中发生跨膜压高报警的56例次,分析报警原因,并进行相应处理。

结果56例次患者进行治疗时跨膜压高报警原因依次如下:脱水过多53例次,管路或透析器凝血1例次,透析器的选择不当2例次。

结论加强巡视,及时找出报警原因并进行相应处理,可确保透析顺利进行。

血液透析治疗时,将患者的血液和透析液同时引进透析器的内外侧 [ 1 , 2 , 3 ]。跨膜压是指透析器中空纤维膜(半透膜)两侧(内外)的压力差,即血液侧与透析液侧的压力差,是透析器要完成治疗时所设定超滤率必须的压力差。正常情况下为正值,但如果跨膜压增高超过透析膜所承受的压力会导致破膜。血液透析过程中,透析机检测系统发现治疗参数异常变化,会及时发出报警,报警如处理不当可引起严重的并发症,不仅影响透析质量,甚至导致患者死亡。本研究就透析过程中发生频次较高的跨膜压高报警,积极分析原因,采取应对措施,取得了满意的效果。现报告如下。
