摘要

质子泵抑制剂(proton pump inhibitors,PPIs)作为全球最常用的处方药之一,由于其安全性及抑酸效率高、耐受性好的特点,被临床广泛应用于抑酸治疗。各种PPI拥有相似的作用机制,即通过阻碍胃壁细胞H +-K +-ATP酶发挥其抑制胃酸分泌的作用。然而,越来越多的学者发现,PPIs存在高比例的超适应证和超疗程用药问题,估计全球25%~70%使用处方PPIs的患者并不具备用药指证 [ 1 ] ,而超疗程用药约占30%~50% [ 2 ] 。PPIs的不良反应多轻微、短期,最常见的包括头痛、背痛和胃肠道症状等。近年来,PPIs致肾脏损伤不断被报道,如PPIs可引起急性间质性肾炎(acute interstitial nephritis,AIN),即PPIs相关性急性间质性肾炎(PPIs-AIN),有学者认为PPIs-AIN是临床上急性肾损伤(acute kidney injury,AKI)一个重要但常被忽视的原因 [ 3 , 4 ] 。虽然部分AIN患者在停用PPIs后肾脏功能可恢复正常,但仍有部分患者病情可能发展为慢性肾脏病(CKD),甚至进展至终末期肾脏病(ESRD) [ 3 ] 。PPIs也可能与慢性肾脏病患者的肾功能恶化有关。我们总结了PPIs应用对肾脏的影响及其可能机制,为临床合理用药提供参考。
