摘要

目的探讨旋转DSA技术在头颈部的应用。

方法使用Siemens Multistar TOP数字减影血管造影系统,对疑有血管病变的45例患者首先分别进行常规正侧位DSA检查,而后选择合适的技术参数,对其中18例进行旋转DSA检查。

结果所有18例患者(颅内动脉瘤7例,动静脉畸形5例,颈动脉海绵窦瘘4例,鼻咽部纤维血管瘤2例)病变血管的位置清晰可见,其中15例病变血管的供血和分支动脉避开对比剂的重叠,3例显示效果不好。

结论旋转DSA能够显示头颈部大多数病变血管的供血动脉和分支结构,是对常规DSA的重要补充,对头颈部血管病变的诊断和治疗有很大价值。

ABSTRACT

ObjectiveTo evaluate rotational DSA in vascular diseases of head and neck.

MethodsConventional anteroposterior and lateral views of DSA were initially taken in 45 patients suspected with abnormal vasculature using DSA (Siemens Multistar TOP DSA system) . Among them, rotational DSA was performed in 18 patients in the same DSA unit.

ResultsThe precise location of vascular diseases was clearly visualized on rotational DSA imaging in all 18 cases (intracranial aneurysms,n = 7; arteriovenous malformations, n = 5;carotid-cavernous fistula, n = 4; nasopharyngeal fibroangioma, n = 2) . Overlaping of branches and feeding arteries of the abnormal vessels was avoided in 15 patients using the rotational DSA, but failed in the remaining 3 patients.

ConclusionRotational DSA can demonstrate the details of the targeted vessels. It is a useful examination supplemented to conventional DSA, which may play an important role in diagnosis and treatment of vascular diseases of head and neck.
