摘要

目的引进并汉化疼痛共情量表（the Empathy for Pain Scale，EPS），检验中文版EPS在医学生中的信效度。

方法经过翻译、回译、语义分析、专家咨询及文化调试，最终确定中文版EPS条目。于2018年6 —10月，采用便利抽样法，选取531名在校本科生进行问卷调查。采用相关分析法和临界比值法进行项目分析，采用内容效度、结构效度、效标关联效度进行量表的效度检验，计算Cronbach's α系数、折半信度检验量表的内在一致性。

结果中文版EPS保留原12个条目，析出2个因子，2个因子命名为身心不适反应和移情反应，累计方差贡献率为69.059%。量表各条目的内容效度为0.83～1.00，总内容效度为0.94。中文版EPS总分与中文版人际反应指针量表（IRI-C）得分呈正相关（ r=0.370， P<0.01）；量表的Cronbach's α系数为0.914，身心不适反应、移情反应2个维度的Cronbach's α系数分别为0.935和0.775，量表的折半信度为0.896。

结论中文版EPS包含4个场景、2个维度，具有良好的信效度，可用于测量我国医学生的疼痛共情水平。

ABSTRACT

ObjectiveTo translate the Empathy for Pain Scale (EPS) and evaluate the reliability and validity of the Chinese version of EPS in medical students.

MethodsThe items of the Chinese version of EPS were determined by translation, back-translation, semantic analysis, expert consultation and culture adaptation. From June to October 2018, this study selected 531 undergraduate students by convenience sampling to carry out questionnaire survey. Correlation analysis and critical ratio were used to the item analysis. Validity test was implemented with the content validity, construct validity and criterion-related validity; and internal consistency was tested with the Cronbach's α value and split-half reliability.

ResultsThe Chinese version of EPS had 12 items and two factors named as the body and mind discomfort reactions and empathy reactions with 69.059% for the total variance contribution. Content validity of items ranged from 0.83 to 1.00, and the total content validity was 0.94. The total score of the Chinese version of EPS had a positive correlation with the score of the interpersonal reactivity index (IRI-C) ( r=0.370, P< 0.01) . The Cronbach's α value of the scale was 0.914; Cronbach's α values of two dimensions, the body and mind discomfort reactions and empathy reactions, were 0.935 and 0.775 respectively. The split-half reliability of the scale was 0.896.

ConclusionsThe Chinese version of EPS has four scenes and two dimensions as well as a good reliability and validity which could be used to evaluate the level of empathy for pain among medical students in China.

疼痛共情（Empathy for pain）作为共情较典型的表现形式，是指个体对他人疼痛的感知、判断和情绪反应，也就是对他人疼痛的"感同身受" [ 1 , 2 , 3 ]。疼痛共情能够促使个体理解他人的痛苦并产生适当的情绪和行为反应，增进个体的亲社会行为，从而维持良好的人际关系 [ 4 ]。而缺乏对他人的疼痛共情则可能会出现暴力行为、人际关系恶化等不利于个人和社会发展的问题 [ 5 ]。医务人员若能有效地共情患者的疼痛，有助于建立良好的医患关系，增强患者对医护人员的信任，提高治疗的依从性，从而达到满意的治疗效果 [ 6 , 7 , 8 , 9 ]。目前，国内对于疼痛共情的研究多采用疼痛共情实验范式，其实验实施过程较为复杂，需要投入较多的人力、物力，且实际操作的便捷性较差 [ 10 , 11 , 12 , 13 , 14 ]。澳大利亚学者Giummarra等 [ 15 ]编制的疼痛共情量表（The Empathy for Pain Scale，EPS），可作为评估个体疼痛共情的便捷测量工具。此量表包含情感痛苦、移情反应、替代疼痛3个维度，信效度良好。本研究旨在引进和汉化EPS，并检验中文版EPS在我国医学生中的信效度，以期为了解医学生疼痛共情的现状及其影响因素，更好地开展医学生疼痛共情的教育与培养提出建议。
