摘要

目的探讨急性白血病(AL)患者血清超敏C反应蛋白(hs-CRP)、乳酸脱氢酶(LDH)表达及其临床意义。

方法抽取2013年12月至2019年12月郑州人民医院收治的AL患者150例为AL组，另抽取同期健康体检者50例为对照组。比较两组及AL组中初治期、缓解期、复发期血清hs-CRP和LDH水平，分析hs-CRP、LDH不同水平患者5年生存率。

结果AL组hs-CRP、LDH水平高于对照组( t＝4.066、14.110， P<0.05)。AL组初治期及复发期患者血清hs-CRP、LDH水平高于缓解期( P<0.05)；AL组初治期与复发期患者血清hs-CRP、LDH水平比较， P>0.05。AL患者中hs-CRP高水平者5年生存率为32.29%(31/96)，低于hs-CRP低水平者的42.59%(23/54)，χ ²＝6.764， P＝0.009；LDH高水平者生存率为22.78%(18/79)，LDH低水平者生存率为50.70%(36/71)，两者比较χ ²＝21.010， P＝0.001。

结论AL初治期及复发期患者血清hs-CRP、LDH水平高于缓解期，且血清hs-CRP、LDH高水平者5年生存率低于低水平者。

ABSTRACT

ObjectiveTo investigate the expression and their clinical significance of serum hypersensitive C-reactive protein (hs-CRP) and lactate dehydrogenase (LDH) in patients with acute leukemia (AL).

MethodsA total of 150 AL patients who were treated in Zhengzhou People’s Hospital from December 2013 to December 2019 were selected as the AL group, and 50 patients with healthy physical examination during the same period were selected as the control group. The serum hs-CRP and LDH levels in the initial treatment, remission, and recurrence phases of the two groups and the AL group were compared, and the 5-year survival rates of patients with different hs-CRP and LDH levels were analyzed.

ResultsThe levels of hs-CRP and LDH in the AL group were higher than those in the control group ( t=4.066, 14.110, P<0.05). The serum hs-CRP and LDH levels of patients in the initial treatment and recurrence phases of the AL group were higher than those in the remission phase ( P<0.05); the serum hs-CRP and LDH levels of patients in the initial treatment and recurrence phases of the AL group were compared, P>0.05. The 5-year survival rate of AL patients with high levels of hs-CRP was 32.29% (31/96), which was lower than 42.59% (23/54) of patients with low levels of hs-CRP (χ ²=6.764, P=0.009); the survival rate of people with high LDH level was 22.78% (18/79), and the survival rate of people with low LDH level was 50.70% (36/71). The above two was compared, χ ²=21.010, P=0.001.

ConclusionsThe levels of serum hs-CRP and LDH in AL patients during initial treatment period and recurrence period were higher than those during remission period. The 5-year survival rate in patients with high levels of serum hs-CRP and LDH is lower than that with low levels.

急性白血病(acute leukemia，AL)是造血干细胞的恶性克隆性疾病，患者常表现为贫血、出血、感染和浸润等征象。AL患者常伴有凝血功能异常，有报道指出，急性白血病的凝血功能异常与血液细菌性感染炎症反应密切相关 ^{[

]}。超敏C反应蛋白(hypersensitive C-reactive protein，hs-CRP)是应用超敏方法检测的急性时相C反应蛋白，是常用的炎症指标之一，其血清水平变化与多种急性炎症反应有关 ^{[

]}；乳酸脱氢酶(lactate dehydrogenase，LDH)是一种糖酵解酶，目前认为，其与心血管肿瘤的发生、发展密切相关，因急性白血病患者体内的白血病细胞具有肿瘤细胞的特征，可能会导致LDH的异常表达 ^{[

]}。但目前尚无报道明确指出LDH与AL的关系。本研究探讨AL患者血清hs-CRP、LDH水平及其临床意义，以期为AL的治疗及病情监测提供参考依据。
