摘要

常见消化道肿瘤包括食管癌、胃癌、结直肠癌等,都是发病率和死亡率居前的恶性肿瘤,极大危害人类健康。目前常见消化道肿瘤的筛查技术分为无创性和有创性检查。无创性检查有血清蛋白标志物、粪便隐血试验、液体活检、基因检测、影像组学等。有创性检查即内镜检查是消化道肿瘤筛查的金标准,随着内镜技术的发展,放大内镜结合窄带成像技术、智能分光比色内镜、高清智能电子染色、蓝激光成像、共聚焦激光显微内镜等图像增强技术,以及磁控胶囊内镜的面世,实现了对全消化道开展镜下检查,拓宽了内镜下视野,提高了病变的检出率和诊断准确率。随着基因技术、生物信息、大数据和人工智能的发展,消化道肿瘤筛查将逐渐演变为基于无创性筛查技术构建风险评估模型,建立不同风险人群的随访队列,对高危人群进一步内镜检查,并有效开展地随访监测管理。

我国食管癌、胃癌、结直肠癌等消化道肿瘤的发病率和死亡率分别占各类肿瘤的前6位和前5位,占全球消化道肿瘤病例近50%,且近85%患者在确诊时属于中晚期 1。以胃癌为例,与同为胃癌高发的日本、韩国相比,其胃癌早诊率已达到70%以上,而我国尚不到15% 2。控制肿瘤的发病率和死亡率,必须遏制源头,也就是降低或减少患病的已知危险因素,改变不良生活方式,推行全民普查和高危人群精查,及时高效地发现肿瘤早期病灶和癌前病变,做好零级预防和一级预防,这是改变我国消化道肿瘤防治严峻形势的可行且高效的途径。
