摘要

肾癌发病率位居恶性肿瘤中男性第6位、女性第9位 [1],约90%为肾细胞癌(renal cell carcinoma,RCC),起源于肾小管上皮,包括透明细胞(75%)、乳头状细胞(12%)、嫌色细胞(4%)、大嗜酸粒细胞瘤(4%)和集合管(<1%)等多种组织亚型 [2]。由于影像技术进展,肾癌的检出率逐年升高。临床多种治疗方法已被应用于肾癌,包括开腹和腔镜下肾切除术、保留肾单位肾部分切除术以及开腹、腔镜下和经皮局部消融治疗。自1998年Zegel等 [3]首次临床应用冷冻消融治疗肾癌以来,肾癌的消融治疗成为继腹腔镜手术后微创治疗的又一进展,包括射频、冷冻、微波、激光、海扶消融以及不可逆电穿孔技术均已在临床成功应用,具有疗效肯定、可保留较多正常肾单位、操作简便、患者易耐受、住院时间短、恢复快、并发症少等优点,其中以影像引导射频、冷冻和微波经皮消融应用最为成熟。消融治疗已被中国临床肿瘤学会(Chinese Society of Clinical Oncology,CSCO)肾癌管理指南、美国国立综合癌症网络(National Comprehensive Cancer Network,NCCN)指南、美国泌尿学会(America Urological Association,AUA)指南、欧洲泌尿学会(European Association of Urology,EAU)指南、欧洲肿瘤内科学会(European Society For Medical Oncology,ESMO)指南等国际权威指南推荐用于临床T1a期肾癌治疗 [2,4,5]。目前针对肾癌的消融指南,仅欧洲心血管和介入放射学会于2016年发布了小肾癌(T1a期)经皮消融指南 [6],消融发展至今,技术和临床应用均取得了显著突破和发展,包含T1a及其以上分期肾癌的消融规范指导需要更新和建立。

本指南立足于影像引导肾癌经皮消融治疗,由中华医学会超声医学分会和中国研究型医院学会肿瘤介入专业委员会组织专家制定,目的是为临床医生提供肾癌消融治疗的循证医学证据以及推荐规范治疗流程。指南专家组由多学科专家团队组成,包括影像科、介入科、肿瘤科、泌尿外科、病理科医生。指南基于循证医学基础建立,着眼于影像引导经皮射频、微波、冷冻消融治疗,并参考CSCO、NCCN、EAU、ESMO、AUA肾癌诊疗临床实践指南和欧洲心血管和介入放射学会肾癌消融指南。
