心脏骤停(cardiac arrest, CA)是引起世界范围内患者死亡的重要原因之一。2020年国际复苏联络委员会对全球16个院外CA(out-of-hospital cardiac arrest, OHCA)中心的数据分析显示,OHCA的年发病率为(30.0~97.1)/10万;接受治疗的患者30 d存活率为3.1%~20.4%,仅2.8%~18.2%的患者有良好的神经功能预后 [1]。我国北京、上海等城市报道OHCA患者出院时神经功能良好的患者比例更低 [2,3]。CA患者经过心肺复苏(cardiopulmonary resuscitation, CPR)自主循环恢复(return of spontaneous circulation, ROSC)后脑损伤是其死亡的主要原因 [4],因此ROSC后脑保护一直是治疗的重点和难点。目标体温管理(targeted temperature management, TTM)是应用物理和(或)化学方法将核心体温快速降到目标温度,维持一定时间后缓慢升温至正常生理体温的过程。2002年发表的两个随机对照研究证实 [5,6],CA复苏后脑损伤患者的核心体温控制在32~34℃维持12~24 h,其出院生存率和6个月神经功能预后有所改善。自此开始TTM逐渐成为CA的ROSC后昏迷患者的重要治疗措施,被国内外专家共识推荐为ROSC后昏迷患者脑保护唯一有效的治疗方法 [7,8]。然而,近年来关于TTM的实施也出现了一些争议,本文就TTM在CPR患者中应用的进行讨论。
