摘要

主动脉疾病是一类严重的心血管疾病,包括主动脉夹层、主动脉壁内血肿和主动脉穿透性溃疡等。近年来,随着诊断技术和治疗手段的不断发展,主动脉疾病的诊断治疗取得了明显进步。然而,主动脉疾病的临床异质性和高死亡率仍然对临床实践构成挑战。该文介绍了主动脉疾病的流行病学、诊断和治疗及预后风险评估等方面的研究进展。讨论了主动脉疾病管理的新方向,包括个体化诊治策略、多学科协作模式、机器学习在预后预测中的应用以及长期随访与预后评估体系的建立,并对主动脉疾病的未来研究方向进行了展望。

主动脉疾病的发病率和死亡率均较高,其主要临床类型包括主动脉夹层、主动脉壁内血肿和主动脉穿透性溃疡。近年来,随着影像学技术的进步、分子生物学的深入研究以及临床治疗策略的改进,主动脉疾病的诊断和治疗取得了明显进展。然而,主动脉疾病的临床异质性和高死亡率仍然对患者生存和生命质量构成严峻挑战。
目前的研究表明,基于生物标志物的早期诊断方法、影像学辅助技术以及新型腔内修复手术显著提升了患者的生存率。然而,围绕主动脉疾病的许多基本问题仍未解决,例如如何精确预测夹层的破裂风险、如何优化术后长期监测,以及是否存在更有效的药物治疗措施。与此同时,遗传学研究和人工智能技术的引入也为这一领域带来了新的希望。结合本期“主动脉疾病”栏目发表的3篇临床研究 1 , 2 , 3,本文总结主动脉夹层的最新进展、临床挑战以及未来的发展方向,为临床医师和相关研究者提供参考。
