摘要

目的探讨应用内镜喷洒管置入肺部病灶深部灌注/灌洗以提升支气管肺泡灌洗液(BALF)阳性率及临床疗效。

方法采用回顾性研究,选择2023年5月至2024年5月在华中科技大学同济医学院附属武汉市中心医院新洲院区呼吸内科因肺部感染接受支气管镜治疗的患者88例,根据治疗方法不同分为对照组及观察组,各44例,对照组采用传统方法即将支气管镜嵌合好靶向支气管,再经活检孔快速注入0.9%氯化钠注射液,每次25~50 mL,总量100~250 mL。观察组采用改良方法即将支气管镜插入靶支气管后,应用一次性使用内镜喷洒管插入病灶深部,喷洒0.9%氯化钠注射液20~30 mL,然后快速回吸约40%以上的BALF。观察比较两组BALF的阳性率、患者的耐受情况及治疗效果。

结果观察组BALF中结核、细菌、真菌、病毒及支原体衣原体阳性率[25.0%(11/44)、52.3%(23/44)、31.8%(14/44)、29.5%(13/44)、22.73%(10/44)]均高于对照组[9.1%(4/44)、27.3%(12/44)、11.4%(5/44)、9.1%(4/44)、6.8%(3/44)]( χ 2=3.94、5.74、5.44、5.91、4.42,均 P < 0.05);观察组患者在治疗后发生心悸、胸闷、面色苍白[22.7%(10/44)、29.5%(13/44)、22.7%(10/44)]均少于对照组[68.2%(30/44)、84.1%(37/44)、68.2%(30/44)]( χ 2=18.33、26.68、18.33,均 P < 0.001);观察组患者住院时间以及住院费用均低于对照组( t=4.27、11.16,均 P < 0.001)。

结论改良方法可以提高肺部结核、细菌、真菌、病毒及支原体衣原体等的阳性率,降低并发症,并且可以减少住院天数及降低住院费用。

有报告指出,玻璃瓶产生的微异物通过医源性操作进入肺部,有可能会给患者带去可能未知的伤害 1 , 2。应用内镜喷洒管及塑料瓶包装的利多卡因、0.9%氯化钠注射液来预防,并且通过内镜喷洒管针对性取材及用药取得了一定的成绩。内镜下取支气管肺泡灌洗液(BALF)是一种安全、相对无创的方法,已经成为肺部疾病诊断和治疗的一种重要手段 3,1974年Reynolds等首先在临床上应用BALF对患者下呼吸道进行灌洗。我国学者于1986年起也开始进行这方面工作 4,到1994年我国建立了人的支气管肺泡灌洗技术标准化操作规程 5,从此BALF在临床上得到广泛应用。气管镜由于操作方便在临床上的应用越来越广泛,尤其是在肺部疾病的治疗中,同时也进一步提高了肺部疾病的诊断率 6。随着肺部疾病治疗手段增多、抗生素广泛应用、反复感染 7,导致病情难以控制,早期明确病原体有着极其重要的意义。近年来随着高通量测序技术(NGS)、宏基因组学二代测序技术(mNGS)、聚合酶链式反应(PMSEQ)等病原学PCR技术的临床普及 8,对于标本的选择有了更高的要求,改良BALF方法对临床诊治有着重要意义。
