摘要

目的探讨针对性健康教育对下肢骨折术后深静脉血栓的预防效果及对患者生活质量的影响。

方法该研究为前瞻性对照研究,选择嘉兴市第二医院2023年10月至2024年2月择期行手术治疗的下肢骨折患者60例,采用随机数字表法分为对照组与研究组,各30例。研究组给予针对性健康教育,对照组给予常规健康教育。比较两组患者满意度、深静脉血栓发生率、患侧皮肤异常情况、干预前后踝关节活动度和生活质量变化。

结果两组患者总满意率差异无统计学意义( P > 0.05)。研究组患者深静脉血栓发生率[3.33%(1/30)]低于对照组[26.67%(8/30)]( χ 2=4.71, P < 0.05)。研究组患者患肢皮肤异常率[13.33%(4/30)]低于对照组][40.00%(12/30)]( χ 2=5.45, P < 0.05)。研究组患者干预后踝关节活动度[(62.34±8.85)°]高于对照组[(49.98±7.97)°]( t=5.68, P < 0.05)。研究组患者干预后躯体功能[(74.36±5.15)分]、心理功能[(71.23±5.35)分]、社会功能[(78.81±3.87)分]和物质生活[(82.16±6.43)分]均高于对照组的(65.46±3.84)分、(60.72±4.08)分、(67.48±5.16)分和(73.42±5.57)分( t=7.59、8.56、9.62、5.63,均 P < 0.05)。

结论针对性健康教育对下肢骨折术后深静脉血栓患者应用效果良好,可降低深静脉血栓形成,改善患者踝关节活动度和生活质量。

深静脉血栓主要指深静脉血液由于多种原因出现异常凝结 1 , 2。调查显示,下肢骨折术后深静脉血栓发生率较高,通常在术后3~5 d内出现,常见临床表现为下肢浅静脉扩张、肿胀、疼痛及皮炎等,一旦出现深静脉血栓,还可能伴器官组织功能障碍 3。因此,针对下肢骨折术后患者采取及时有效的预防,可有效降低深静脉血栓的形成 4 , 5。针对性健康教育是在传统健康教育基础上发展的具有精细、全面等优势的一种新型健康教育方法,用于下肢骨折术后患者可取得良好效果 6。该研究探讨针对性健康教育对下肢骨折术后深静脉血栓的应用效果及对患者生活质量影响。
