外周静脉-动脉体外膜肺氧合辅助期间氧合管理面临的挑战

摘要

在临床实践中,静脉-动脉体外膜肺氧合(venoarterial extracorporeal membrane oxygenation, VA-ECMO)常作为顽固性心源性休克及自主循环难以恢复的心脏骤停患者的挽救性治疗手段 [1]。根据体外生命支持组织(extracorporeal life support organization, ELSO)的登记数据,VA-ECMO辅助期间患者常表现出高氧血症 [2]。高氧血症可加剧细胞氧化应激和炎症反应,导致再灌注损伤、急性肺损伤、急性肾损伤以及冠脉和脑血管的收缩 [3,4]。多项研究指出,高氧血症是导致VA-ECMO患者病死率增加的关键影响因素,尤其在自主循环难以恢复的心脏骤停患者中 [2,5,6,7,8,9]。因此外周VA-ECMO辅助期间的氧合管理仍为临床实践中的一个重大难题。首先,监测指标与技术的局限性以及不同监测部位的氧合差异构成了氧合监测领域的多重挑战;其次,目前尚缺乏统一且明确的最佳氧合目标范围,加之VA-ECMO患者的病因多样性,不同病因导致的氧合障碍机制各异,这可能需要采取不同的氧合管理策略和制定个体化的氧合目标,使得氧合评估难度增加。此外,外周VA-ECMO辅助期间患者的氧供影响因素复杂多变,并且常常呈现动态变化,维持患者氧供与氧耗之间的平衡,进行动态氧合评估亦颇具挑战;最后,通过调节ECMO流量参数及吸入氧浓度等手段来动态精准管理氧合亦存在难度。本文就临床上外周VA-ECMO辅助期间氧合管理所面临的挑战进行阐述。
