近30年来,尽管许多国家的龋病患病率逐年下降,但龋病仍然是世界上最流行的疾病,影响着数十亿人口 [ 1 ] ,在全球产生巨额的医疗费用 [ 2 ] 。因此,如何管理龋病以减少其对人类的影响,已经成为口腔医学领域的重要课题。为实现这一目标,制定龋病管理策略时需要有充分的临床证据做指导。目前,已有越来越多的临床研究证据支持在此基础上形成推荐规范 [ 3 ]

近30年来,尽管许多国家的龋病患病率逐年下降,但龋病仍然是世界上最流行的疾病,影响着数十亿人口 [ 1 ],在全球产生巨额的医疗费用 [ 2 ]。因此,如何管理龋病以减少其对人类的影响,已经成为口腔医学领域的重要课题。为实现这一目标,制定龋病管理策略时需要有充分的临床证据做指导。目前,已有越来越多的临床研究证据支持在此基础上形成推荐规范 [ 3 ]
传统观念认为应完全去除龋坏组织,然后进行牙体充填治疗或粘接修复。在过去的30年,虽然有很多相关文献,但对于何时使用何种去龋方法,答案一直不明确,甚至令人困惑。另外,同一种方法在不同的国家也有不同的解释。随着对龋病认识的深入和大量去龋方法临床试验证据的出现,传统的去龋方法需要进一步改进。
为解决龋病管理防治中出现的这些问题,2015年2月来自12个国家的21位专家相聚比利时,成立了国际龋病共识协作组(International Caries Consensus Collaboration,ICCC )。这些专家包括龋病学、材料科学、临床研究、系统回顾、指南制定等方面的专家,他们同时也是口腔医生。ICCC就龋病相关术语、去除龋坏组织两个方面达成专家共识(consensus recommendations),并全文发表于2016年国际牙科研究协会的会刊 Adv Dent Res。关于本推荐规范的形成、龋病术语推荐规范的详细介绍和讨论,以及关于去除龋坏组织推荐规范的详细介绍和讨论,请参阅Frencken等 [ 4 ]、Innes等 [ 5 ]及Schwendicke等 [ 6 ]2016年发表的文章,本共识根据以上3篇文献编译而成。
