摘要

自20世纪40年代青霉素应用于临床以来,人类对抗肺部感染取得了显著成效。但近年来随着耐药菌的出现,静脉等全身给药方式对于呼吸机相关性肺炎(ventilator associated pneumonia, VAP)等急性感染难以取得满意疗效,同时,支气管扩张症及囊性肺纤维化等结构性肺病的病原体往往难以清除,导致反复、慢性感染,且长期使用抗菌药物带来的细菌耐药问题更加大了治疗难度。雾化吸入抗菌药物等局部给药方式可提高感染部位的药物浓度,有助于提高疗效,因此日益受到重视,但临床应用尚不规范。现介绍下呼吸道局部抗菌药物应用的历程及研究进展,在此基础上提出使用的指征和注意事项。
